ATGenius Instructions for Student-Athletes
This is required to be complete before participation in your sport!
1.Go to atgenius.com:
2. Click on Students/Athletes Login Here
3.Click New Student/Athletes
4. Type in DominicanStars in School Code
5.Select Dominican University
4.Type in Date of Birth and Last name
5. Use your Dominican Email address and choose any password you like.
6. Fill out all the fields under the Student Info tab. Including name, address, phone number, sport, grade level (if you're a transfer just choose the grade level you're coming in as), and any medical conditions I need to be informed about (i.e., asthma, severe allergies, or diabetes, etc.) Don't forget to save.
7. Click the Parent Info tab. Fill out all fields of your emergency contact. In parenthesis next to the guardian name, please add their relation to you. Example: Jane Doe (aunt). Please add both guardians if able. If you are unsure about their employer address and number, please leave it blank. At least try and get the employer name if able. Don't forget to save.
8. Click the Insurance tab.
For those who have insurance: Please upload the front and back of your insurance card by clicking add new insurance card. There is a green plus button on the top right of the pop up screen to add more than document if needed. Once uploaded, fill out the fields under Primary Insurance Information. I need the name of the insurance company, address, policy/ID number, and phone number. If you do not have a group number or primary care physician you may leave those sections blank.
For those who do not have insurance: Please just write in the insurance company field uninsured. This lets the medical staff know to take out an insurance policy for you to play.
Please ignore the Secondary Insurance Card as you do not need it unless you do have two insurances of your own. Don't forget to save.
9. Click the Physical tab. Click upload physical and click the document which has your physical. In the comment section please write 2025. Please Save. Nothing else needs to be done in this tab.

Please make sure your physical is done on this sheet and is signed by an MD or DO. It will not be accepted otherwise.
10. Click the Medical tab. Please check off if you have had or currently have any of the medical conditions listed. If you checked yes to any of them, under Medical Condition Details please give details. For example, if I checked allergies, I would write: I have seasonal allergies that I take Zyrtec for. If I checked surgery, I would write: I had surgery in 2018 for my right ACL.
Under Medications, please any and all medications you are taking. If you are on vitamins or supplements, please add that as well. The NCAA does require that if you are taking a banned substance for medical reasons that there must be documentation. https://www.ncaa.org/sports/2015/6/10/ncaa-banned-substances.aspx This is the website that has the list of the drugs that are banned by the NCAA. If you are taking a substance on this list, please reach out to me at vperez4@dom.edu to discuss next steps.
Under Family History, check if your family has a history of any of the medical conditions listed. Don't forget to save.
11. Lastly, Click the Preseason Docs tab. Please read and sign all forms excluding the sickle cell trait waiver form. The medical history form must be filled out either electronically or by pen and paper and signed. You will have to re-upload the filled out version of the medical history form by pressing start next to the medical history button and then clicking next step and then the add media button.
If you are a first-year or transfer, you must upload your sickle cell trait results. To upload: click on the add documents button at the top and then choose the file that contains your sickle cell trait results, under document type, choose other and then lastly in the comments write: Sickle cell results 2025. Don't forget to save.

1. Go to dom2.atsusers.com: Athlete ID: NEW Password: NEW
2. Select Athlete Information.
3. Change Athlete ID to your Student ID number (last 6 digits on the back side of your student ID card)
4. Change your password to something you will easily remember.
5. Fill-in the general athlete information page, be sure to fill-in all yellow boxes (required). Your year in school should correlate with your participation status as of Fall 2021. If you do not have any medical alerts or allergies, please put None or N/A in the yellow box provided.
a. Click save athlete information (you will be prompted to complete any missed boxes)
b. Once complete, select verify athlete information (clicking Verify assures all information is saved in our database)
6. No information is required under the Immunizations/Paperwork tab.
7. Next, go to the Insurance tab! Please submit new insurance information, select add and fill in all insurance fields (be sure to fill in ins. Phone, ID#, Group# and policy holder information). (Note: for Payor # please type 1 on the Payor # box, as this is your primary insurance information.) If you do not have primary insurance, please select add Commercial Travelers Insurance company select Medical PPO for the Insurance Type.
a. Click Verify Insurance Information. If at any time you need to update or edit your insurance information, simply click on the 1 in the left column and select edit and make appropriate changes.
b. A copy (front and back) of your insurance card must be uploaded directly on this page. If you do not have scanning capability, please e-mail a copy of your card, front and back to vghilardi@dom.edu or mail to Dominican University Athletic Training 7900 W. Division Street, River Forest, IL 60305. You are required to provide this before participation in your sport.
8. Select the Contact tab (Emergency contact), select add to input a new emergency contact and fill in all yellow fields, Save and Verify.
9. Click on the Forms Tab. Then click the drop down arrow next to the Form Name box on the bottom half of the page. You will need to complete each form in this drop down box. You can do this by selecting any form and click new. Repeat this step until all forms are complete. Follow the instructions on each form and fill out completely, Save and Verify. Forms included: General Information, Medical History, Assumption of Risk, Consent to Participate, Consent to Treat, Student Athlete Authorization, Prescription Medication Disclosure, and Sickle Cell Trait, and COVID-19 Screening. If you have sickle cell trait test results, please scan and save your results onto your desktop and upload the form under the e-files tab. Description should say "Sickle Cell Trait", then choose the file saved on your desktop and hit upload. If you do not have scanning capability, please mail a copy of your Sickle Cell Trait results to Dominican University Athletic Training 7900 W. Division Street, River Forest, IL 60305. You also have the option to sign the Sickle Cell Trait waiver.
10. Under the e-files tab, you can look to see that all of your previous forms have been completed. If any forms are missing, please go back to the form tab and select Verify Student Information.
11. If you are taking any ADHD medications, please contact Tori Ghilardi at 708-524-6226 for required documentation. Your physician will need to provide pertinent information regarding your diagnosis and prescription.
12. Download the physical form (DOCX).
Print, take this form with you to your physical appointment with your doctor, then scan your physical form directly to this page and save as "2021-2022 Physical", then choose the file saved on your desktop and hit upload. If you do not have scanning capability, please mail a copy of your physical to Dominican University Athletic Training 7900 W. Division Street, River Forest, IL 60305.
