Q: What forms do I need to fill out before arriving at Dominican University as a student athlete?
A: All student athletes must complete required medical paperwork prior to participating in team practices, scrimmages, or game. Students will create an account on our online platform, ATS, and all forms will be electronically signed there. All paperwork must be on file with the athletic training department before you will be cleared for participation in Dominican University intercollegiate athletics. The link to the ATS portal, as well as instructions, can be found at the top of the page.

Q: Do I have to complete a medical physical before I participate in Dominican University athletics?
A: Yes, you must complete a Pre-Participation Physical Examination before participating in any of Dominican University's intercollegiate athletic programs. The physical must be signed by a MD or DO. High School Physical Forms will not be accepted. Physicals from Walgreens or CVS will also not be accepted. The physical form can be found by clicking here (PDF).

Q: Who makes the return to play decisions?
A: All final decisions on athletically related physical activity will be decided by the athletic training staff and, when necessary, in conjunction with the team physician/s, along with communication and feedback from the student-athlete. The policy will be applied to the following situations during both the traditional and non-traditional playing seasons: sports related daily activity, return to practice protocol, final return to practice clearance, return to competition protocol, and final return to competition clearance.

Q: If I receive a medical bill from an injury I obtained while competing for Dominican University intercollegiate athletics, who do I contact?
A: It is recommended that student athletes have their own health insurance to cover general medical problems and illnesses. If an athlete sustains an injury during a Dominican University Athletics- sponsored activity (practice, strength and conditioning, competition) and requires medical care outside of what can be provided by athletic training staff, the student's (or parents') insurance will act as the primary policy. The Dominican Athletic Insurance through First Agency will provide secondary coverage for remaining charges. In order to use the First Agency policy, the student athlete must fill out a claim for with Athletic Training Staff. This policy will only cover Dominican Athletics-related injuries. It will not cover illnesses or injuries sustained outside of intercollegiate athletics. Dominican University and First Agency can never guarantee that all expenses will be paid for.
If you receive a bill from a Dominican Athletics-related injury, it must be submitted to Athletic Training staff within 30 days. It is preferred that you do this in person, but bills may also be faxed or emailed to the head athletic trainer.

Q: What if I want to see my own Physician for injury/illness evaluation?
A: Each student athlete has the right of seeing their own physician for evaluation, but it is recommended that the student-athlete is first seen by Dominican's team physician. Dominican has a longstanding relationship with Romano Orthopedics, which allows for greater ease of communication and quicker scheduling. When a student-athlete goes to a doctor, a medical note is required in order for the student-athlete to participate in their respective sport.
